Disconnect cause 11 means "loss of modem carrier". Carrier loss may happen for a variety of reasons and many of them are perfectly normal disconnects. You get d=11 if the user has call-waiting and someone calls him while online. This was a killer until we knew to tell those users to turn off call waiting. It may also happen if there is noise on the line and the modems can't retrain in time. You need to take a look at the progress codes to see where are the calls getting stuck.
