Warning 195 is indicative of either a bad modem, a large number of bad calls, or a crappy ass lucent bug. If you issue the diagnostic command "avm" (or "lanp modem" for taos > 8.x.x) you will see the internal modem lists that the max keeps. Whenever a modem exceeds a certain threshold of bad calls the max moves it to the suspect list. If the modem or other modems on the same card take more bad calls the max moves it to the pending 195 list. It then prevents these cards these modems are on from taking any more calls and it waits for every user on that card to disconnect, at which point it resets the modem card. This can cause severe problems if you have the same number of incoming lines as modems. Lucent has yet to release a reliable fix for the problem. TAOS 9.x supposedly addresses the issue but it has enough other bugs of its own to make up for it. 7.0.22 is the only code I've found which doesn't appear to be terribly affected by the problem (but I've not tested this extensively yet).
