I few questions: -
1) Which Radius server?
2) Are the shared Radius secrets the same on the NAS and the RADIUS server?
3) Can the Radius server ping the IP address that the NAS is sending packets
from?
4) Duplicate IP address ?

It looks like the NAS is sending accounting packets to the radius server.
The radius server logs them, then sends a response.
The response is either not getting back, or the shared secret does not
match.
So the NAS resends them.
