Hi Jan,

Fatal Index 1 is almost always a hardware error. I say almost because it
could be software as well. This is the GENERAL fatal error that is not real
specific on what could be wrong. I would start by changing codes. If that
does not work, start to see if perhaps you have a defective modem card but
chances are that you will experience reboots in the near future if you have
not seen them yet.

Thanks,

Subject: (ASCEND) Max 4048 - Fatal Error

We are seeing the following error on six 4048s -- all in different
locations:

FATAL ERROR: Index: 1 Load: ftk.m40 Revision: 6.1.3
         Date: 12/24/2001. Time: 11:59:15
         Location: b001d328 b001dac4 b001db44 b019258c b0194a5c b0072230

or

FATAL ERROR: Index: 1 Load: ftk.m40 Revision: 6.1.3
         Date: 06/01/2001. Time: 22:14:06
         Location: b001d328 b001dac4 b001db44 b01925ac b0196434 b0078ed8

A search on index 1 error is inconclusive. Can anyone provide information
regarding the location information that might shed some light on the units
reloading? Tia.
