If it wasn't for your first sentence, I'd have no trouble recommending  
Request Tracker (http://bestpractical.com/rt/). I've used it for  
several years with great success. It requires a certain investment to  
install and configure, but once it's rolling I think you'd find it to  
be flexible and powerful.
